Documents Alabama will ask you for
You do not need all of these before you file โ filing early protects your start date โ but every one of them will be requested before a decision is made.
1. Proof of identity
A Alabama driver's licence, state ID or passport for each person applying. A birth certificate works for children.
2. Social Security numbers
Required for every person who is applying for coverage, but not for household members who are only counted for household size. A non-applicant parent does not have to give one.
3. Proof of citizenship or immigration status
A US passport or a birth certificate proves both identity and citizenship at once. Lawfully present immigrants generally serve a five-year wait for full Alabama Medicaid, though children and pregnant women are exempt in many states โ and emergency Medicaid covers labour, delivery and emergency care regardless of status.
4. Proof that you live in Alabama
A lease, utility bill or piece of official mail. Alabama Medicaid is state residency, not citizenship of the state โ you qualify from the day you intend to stay, with no waiting period.
5. Current income for everyone in your tax household
Pay stubs, a benefit award letter, or self-employment records. Alabama Medicaid uses MAGI โ your tax household, not the people under your roof โ so a roommate's income is irrelevant and a tax dependant's counts. The limit is $387 a month for a household of three at 18% of poverty.
6. Details of any other health coverage
An employer plan, a Marketplace policy or Medicare. Having other coverage does not automatically disqualify you; Alabama Medicaid can pay second, and people on Medicare should also be screened for the Medicare Savings Programs.
7. Medical bills from the last three months, if you have them
Alabama Medicaid coverage can be backdated up to three months before the month you applied, as long as you were eligible then. Unpaid bills from that window are the reason to file even if you no longer need care today.
8. Proof of a qualifying category, if you are not income-eligible on your own
In Alabama this matters more than anywhere: without expansion, pregnancy, disability, age 65+ or dependent children are what create a pathway at all. Bring the proof โ a pregnancy confirmation, an SSA award letter, or your children's birth certificates.

What Alabama will NOT ask you for
- No asset or resource test. Income-based Alabama Medicaid eligibility has had no asset limit since 2014, so savings, a car or a house do not disqualify you. (Long-term-care and 65+/disabled pathways are the exception and do count assets.)
- No open-enrolment window. Unlike a Marketplace plan, you can apply for Alabama Medicaid on any day of the year, and coverage starts the first day of the month you applied.
- No premium in most cases, and no cost for children's preventive care. Cost-sharing where it exists is capped at 5% of household income.
Filing and timing in Alabama
- Apply through MyDHR (medicaid.alabama.gov), run by Alabama Medicaid Agency, or by phone on 334-242-1700.
- Alabama has 45 days to decide most Alabama Medicaid applications and 90 days when a disability determination is needed; 15โ45 days is the normal range in practice.
- Apply for children separately even if you are denied. Children are covered far above the adult limit through Alabama Medicaid and CHIP in every state, Alabama included, and a household denial does not carry over to them.
- If you are turned down because Alabama has no expansion pathway and your income is at least 100% of poverty, the Marketplace will offer a heavily subsidised plan instead. Below that, ask a federally qualified health centre about sliding-scale care.
